Patient outcome survey for the Care Quality Commission 2011
Patient Satisfaction Survey
As of April 2011 it has become a legal requirement that all dental implant practices which are in a primary care setting are registered with the Care Quality Commission (“CQC”). The CQC stipulate that registered practices must carry out audit including patient surveys and feedback. At Onward House we instigated a survey on patient experiences and satisfaction. The survey was distributed to patients who had completed their implant treatment at the clinic. Patients were all invited to return their completed forms by post.

Reproduced below in order are the first 50 replies returned. These are all unedited except for minor editing where names were given without any inferred permission. Each reply was been tagged with the unique patient referral number in order that the outcome can be verified as genuine to the CQC (Care Quality Commission) and our governing body the GDC (General Dental Council). Where permission was included on the completed survey form the patient name has also included in the permitted format.

We asked 6 key questions (clicking on the question accesses the feedback):

  • How did you come to choose our clinic for your treatment requirements?
  • How did you find the premises and facilities at the clinic?
  • What was your experience of the level of care provided and your overall treatment experience?
  • How well did the treatment outcome meet your expectations?
  • Would you recommend similar treatment to a colleague or friend?
  • Did you feel that the all aspects including the costs of your treatment were clearly explained before you consented to proceed?
The survey sheet also included the following:
  • Please feel free to make any other comment including suggestions about how we might be able to improve our service.

Reproduced below are two of the many cards that we have received to date from patients who completed their implant in 2011 (various others are reproduced in the testimonial section).

Helen lives and works in London and was a victim of a road traffic accident - losing her front teeth at an early age. Her parents were supportive at her initial consultations and throughout the treatment process.
